Guide to Completing Forms on Remini
 

To continue, you must have the Remini App downloaded on your phone or computer.  Forms are most easily completed when using a computer.  

Directions:

1. Download and sign in to the Remini App.

2.  Click on the three horizontal lines in the top left corner.

3.  Select Forms from the drop down list and begin completing the necessary forms as stated below.​

All Children

  • Emergency Form

  • Consent to Administer

  • OTC Medication Form

  • Permissions 2025

  • Vaccine Verification Survey​​

  • Allergy Action plan (if applicable)

  • Medication Form (if applicable)
